There are a lot of interesting clothes that state they're compatible with say, Victoria 4, or Stephanie 4, so on. If I have Genesis and V5, can I still use the __4 compatible clothes? Is there a big difference?
Yes, up to a point. AutoFit will convert them for Genesis, but not everything will convert equally well (in particular, things with extra bones - such as skirts, or long belts, lose their extra bones). This would help with at least some of the troublesome items http://www.daz3d.com/sickle-rigging-and-morphing-system

Is there any plan to update Auto-fit at some point? It's a bit primitive in the selections of clothing types it offers. Usually I have to select something close, but not actual. e.g how come there's no "shoe" option? Pretty basic, surely?

AutoFit was updated in the past (I believe this was going from DS4.0 to DS4.5) and its functionality improved with that change, but I have no idea whether another such updated is planned for the future. The Sickle Rigging and Morphing Kit Richard linked to might be of interest if you want better results than AutoFit delivers. It's not a one-click solution, but it does come with a manual.
Shoes are another thing that AutoFit doesn't convert very well...apparently there are some significant differences between Genesis feet and V4/M4 feet...
