Can one make genesis look middle aged or old? Or does one have to purchase that as some kind of add on?

you need an add on. these are custom morphs that are an update of the old Gen 4 aged transferred with GenX

TIP: the Aged for M4 and V4 sets are mostly done with TEXTURES. You can use the texture Maps on Genesis to do this...

Recently did a render of a Neil Gaiman short story about an 18-year old runaway who meets her future middle-aged self who is described as "plump" and kind. I tried the V4 morphs and they were good, but I wanted both the old and the young selves to have a stronger resemblance.

I used Zev0's facial wight and aging morphs and was totally happy with the result:
