Did anyone else fill out a Daz survey for $5 in store credit?

Did anyone else fill out a Daz survey for $5 in store credit? I did, but the credit doesn't show up anywhere that I can find. Not in 'store credit', and not in 'gift cards.'  I know I filled it out because the top bar of the Daz store still has the banner 'Earn Store Credit for Your Feedback,' and when I click on it, it tells me I already took that survey.  How do I get my five bucks?
