    I had a problem with how I was submitting the form (if you don't provide a small 300x350 pic it won't submit)

    It's up, pending approval, but it can also be downloaded from the Google Drive link in the first post. ShareCG seems to be super busted, I mailed various admin contacts but they probably already know (plus it's sunday)
